Florida Now Requires EKGs for High School Athletes — Here's What Families Need to Know

Florida just became the first state in the U.S. to require that high school athletes receive an electrocardiogram (EKG) before participating in sports. The new law, known as the “Second Chance Act,” takes effect on July 1, 2025, and will be enforced starting with the 2026–27 school year.

The goal of this new law is to improve detection of potential heart conditions that could result in sudden cardiac death in otherwise asymptomatic, young, active, well-appearing athletes.


What Does the New Law Require?


While the law is effective July 1, 2025, the FHSAA (Florida High School Athletic Association) will not enforce the requirement until the 2026-2027 school year. The requirement requires a screening EKG for all public high school athletes in grades 9-12 participating in interscholastic athletic competitions, as well as "practices, tryouts, workouts, conditioning or physical activity" . The screening EKG must have been completed at least once in the two years prior to participation. The law also requires that schools must provide access to low-cost testing (EKG's must not cost >$50) or offer an opt-out for medical or religious reasons. By the 2028-2029 school year, the FHSAA must have a formal policy that prohibits a student-athlete with an abnormal EKG from participating in tryouts, practice, or competition.


What Is an EKG and Why School Athletes?


An electrocardiogram (EKG or ECG) is a quick, painless test that records the electrical activity of the heart. Small sensors (electrodes) are placed on the chest, arms, and legs to detect the heart's rhythm and patterns.


The test can identify:

  • Abnormal heart rhythms (arrhythmias)

  • Enlarged heart chambers

  • Conditions like hypertrophic cardiomyopathy—a common cause of sudden cardiac death in young athletes

High school athletes are specifically targeted because intense physical activity can trigger life-threatening heart events in teens who may not show any symptoms during regular daily life. By screening before participation, schools and families can prevent tragedies on the field, court, or track. The entire test takes less than 10 minutes, and results are reviewed promptly by our medical provider.


What Happens If an EKG Shows a Concern?


If your child’s screening EKG shows something abnormal or unclear:

  1. The medical provider will review the results with you and explain what the findings mean.

  2. A referral may be made to a pediatric cardiologist for further evaluation, such as:

    • Repeat EKG

    • Echocardiogram (ultrasound of the heart)

    • Exercise stress test

    • 24-hour heart monitor (Holter monitor)

  3. Participation in sports will be paused until a cardiologist determines it is safe for your child to return.


These next steps are precautionary—and in many cases, findings are minor or benign. But the screening provides critical peace of mind for families and coaches alike.
